👉 One of these forms is required for counseling and nursing services. This form must be completed in order for our nurses to provide any care for your child—including basic first aid.❤️🩹
This update is part of the new SB12 bill, which goes into effect Monday, September 1st.
✅ Please take a few minutes to complete the forms as soon as possible so we can continue supporting your child’s health and well-being at school.
